A transitional housing program that serves homeless individuals and families who have been diagnosed with a serious mental illness. Some clients may also be chronically homeless, domestic violence victims and/or have substance abuse issues. Comprehensive case management services are offered to maintain psychiatric stability, secure transitional housing and ensure individuals are able to maintain or increase their income, from all sources, and exit, within 24 months, to permanent housing (with supportive services as necessary). The program has the capacity to serve up to 24 households through the use of 16 apartment units annually.
