Community-Based Services

Pioneer Center is dedicated to developing and growing the community through Home Based Support Services as well as Client & Family Services. By providing these services we are able to help individuals and families utilize community resources, navigating funding, and to maintain independence in their lives.

Home Based Support Services

Home Based Support Services are provided to families and individuals with an intellectual or other developmental disability who have received an Illinois Department of Human Services award letter that states that they have access to funding for services provided by the Home Based Support Services.

Pioneer Center for Human Services offers a self-direction assistant to assist individuals and families with navigating the Home Based Support Services Program. The self-direction assistant is also involved in the Person Centered Plan process which outlines the client’s needs, wants, and services.

Client & Family Services

Supportive Living enables a person to live in his or her own apartment, with support and assistance from the Pioneer Center team. Staff help individuals build the skills they need to live independently, such as budgeting, grocery shopping and housekeeping. Our team also provides medical support, such as medication management and scheduling of doctor visits.

Admission Criteria

To be considered for admission to the Client and Family Support Program, an individual must:

  1. Be eligible for SSI or SSDI
  2. Live in his or her own home
    Own home includes:

    • Living alone or with chosen companions.
    • Living with parents, foster parents, legal guardian, or other relatives
  3. The individual served in the CFS program shall be at least 18 years of age, have a developmental disability and need an array of services and a supervised living arrangement.
  4. The individual or guardian shall give informed consent to participate in the CFS Program, which shall be documents in the individual’s record
  5. The individual or guardian shall agree to participate in the development and implementation of the Individual Service Plan.

The CFS program is open to individuals that reside in McHenry County.
