Benefits

United Healthcare – We offer four options: 2 HMO options, 1 PPO option, and HDHP with a HSA (Health Savings Account).

MetLife– Two options: Low Plan and High Plan. Both offer In-network and Out-of-network coverage. High Plan includes orthodontia for covered members.

We offer two plans, EyeMed Vision and VSP Vision.

EyeMed Vision provides a $10 copy for exam. Frames have $120 allowance every 24 months. Standard plastic lenses have a $25 copay every 12 months. Contact Lenses have a $135 allowance every 12 months in lieu of standard plastic lenses. Major retail chains are included in the network (e.g. Walmart, Target, LensCrafters).

VSP Vision provides a $10 copay for exams. Frames have $130 allowance every 24 months. Standard plastic lenses have $25 copay every 12 months. Contact lenses have a $130 allowance every 12 months in lieu of standard plastic lenses.

Long-term – after 60 calendar day elimination period pays 60% of earnings on a monthly basis to a maximum of $6,000/month. Premiums are based on earnings and age.

Pioneer Center provides coverage with a benefit amount of 1.5 times annual salary. Includes accidental death and dismemberment benefit of one times your annual salary. Employee-paid life insurance for self, children and spouse are also available.

You may elect to have pre-tax dollars deducted to pay for dependent care and/or unreimbursed medical expenses. You must be employed before the 1st of any calendar year to participate in that year’s plan. You must reenroll each year you would like to participate.

You may elect to have pre-tax dollars deducted to pay for medical expenses if you are enrolled in our High Deductible Health Plan.

One day available after 6 months’ employment; thereafter on anniversary date, two days per year. Unused days do not carry over.

Newly eligible staff begin accruing sick time upon hire and will accrue 5 days during the first year of employment. Staff are eligible to use sick time after the first six months of employment. Beginning with the first anniversary, sick time accrues to 10 days per year. Unused sick days can be accumulated to a maximum of 60 days.

Regular Full Time employees begin accruing vacation time after 6 months of employment, and will accrue 5 days during the first year of employment.

Upon fifth year anniversary, 15 days of vacation accrue.

Upon tenth year anniversary, 20 days accrue.

Beginning after the first 60 days of employment, holiday pay is provided as incorporated in the agency calendar – typically Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving Day, and the day after, Christmas Eve and Christmas Day, New Year’s Eve and New Year’s Day. 1 floating holiday.

In event of death of spouse, child, parent, grandparent, grandchild, brother, sister, or in-laws – 3 days with pay.

Release time at full pay. Jury Duty compensation reimbursed to Agency.

Automatic Enrollment after 30 day eligibility period with a contribution percentage of 3% and auto increase annually to a maximum of 10%. Pre-tax employee contributions to a 403(b) plan; discretionary employer contributions are possible.

The agency covers all employees under a professional liability policy.
