Home-Based Support Services

Pioneer Center’s Home-Based Support Services help individuals and families navigate available resources and create personalized support plans that promote independence, choice, and an improved quality of life. Our Self-Direction Assistants provide guidance to individuals who have received funding award letters from the Illinois Department of Human Services (IDHS) and help them make informed decisions about the services and supports available to them.
